You have the right to request a list of certain disclosures of your PHI made by us, excluding disclosures made for treatment, payment, or healthcare operations.
D. Right to Request Restrictions
You may request restrictions on how we use or disclose your PHI. While we are not required to agree to all requests, we will comply if the request is reasonable and does not interfere with treatment, payment, or operations.
E. Right to Confidential Communications
You may request that we communicate with you in a specific way (e.g., via email or phone) or at a specific location.
F. Right to a Paper Copy
You have the right to request a paper copy of this Notice at any time, even if you agreed to receive it electronically.
4. Our Responsibilities
We are required by law to:
Maintain the privacy and security of your PHI.
Provide you with this Notice of our legal duties and privacy practices.
Notify you in the event of a breach of your unsecured PHI.
Follow the terms of this Notice currently in effect.
5. Changes to This Notice
We reserve the right to change the terms of this Notice at any time. Any changes will apply to all PHI we maintain. The revised Notice will be posted on our website and available upon request.
6. Complaints
